Online Program Request Form

Thank you for showing an interest in Lifesaver 101’s excellent First Aid and CPR programs. Making an online reservation is very simple and takes little time. To finalize your online booking request, the following sequence of events must take place:

  1. This online form must be completed. Please ensure that your e-mail and phone number are correct; otherwise, we cannot contact you.

  2. The Submit Request button must be clicked to send your application to us. A screen message should immediately appear to indicate that your booking request has been transmitted.

  3. You will receive an auto-response e-mail from Lifesaver 101. After opening the e-mail, please review all order information contained therein to verify its accuracy.

  4. You must send a reply e-mail to Lifesaver 101 to confirm that your online order information is correct.

  5. You are required to pay in advance to reserve your seat. After you have paid cash at our offices or made credit card arrangements by phone, someone from Lifesaver 101 will acknowledge that a seat has been reserved for the program of your choice.

  6. Copy the words ‘ONLINE BOOKING REQUEST APPROVED’ from within the bolded paragraph at the bottom of the e-mail, click on the Reply button in your e-mail window and then paste the copied text into the Subject line. Lastly, click Send to transmit your confirmation to sudbury@lifesaver101.com. Alternately, you may click the Reply button, type ‘ONLINE BOOKING REQUEST APPROVED’ in the e-mail text window, and then click Send).
SPECIAL NOTE: If you do not receive a call from us within a day or two of submitting your confirmation e-mail, please contact our training facility at (705) 677-8258 to review your application status. If you are attempting to book on short notice (i.e., for a course starting within the next three days), please telephone us instead of submitting an online application. Thank you again for your interest in Lifesaver 101.
